The optional argument would need to get added to the call of preview,
not the call of xy.

This looks like you are editing a file _region_.tex, or at least that
is the impression AUCTeX has. Very unhealthy. _region_ should never
be edited or compiled by the user manually.
I check the default remedy of calling the _preview_ package with a
"delayed" option: it does not appear like this is what you did from
your description.
