Here is a new version.  The API didn't change, but the error handling
and doc strings did.  There were several large and/or subtle changes
in this version, so I'd appreciate more review.

Does anyone know if any standard guarantee that the characters
A-Za-z0-9+/ must be encoded in the 0..255 interval in 'char', if char
is larger than 8 bits?  Otherwise something like

#if 'A' > 255 || 'B' > 255 || ...
# error character constants too large
#endif

should probably be added.  Or it could be added as an assumption in
README.
