Maybe check about:cache and see if there's anything unusual in there. I'm guessing that maybe if you had disk cache turned on then it might have cached the file sometime. Do you have Private Browsing enabled? Starting Private Browsing won't delete disk cache set beforehand in non-Private-Browsing mode. Try disabling Private Browsing, do Ctrl+Shift+Del and then re-enable. Andrew.
