Gnumach clean-up 9

Hi folks,

Here is a huge patch to fix "implicit declaration of function.."
warnings.

I've added a changelog this time! :)

There are a couple of things I've added that I have reverted (I left the
code in commented out).  If you want to guide me, I'd love to fix them.

They are:
device_pager_setup in dev_hdr.h.
pcmcia_glue.h - Should Debugger be there also???
net_io.h - net_hash_entry_remove. (I may have fixed this one?)
net_io.c - Uses htonl and others that should be available from
asm/byteorder.h.


There are still several warnings but I might need some help on the
remaining.

Let me know if I need to fix anything or made any major mistakes!
