I have two versions of LilyPond running here:  2.1.6 and 2.1.14.  I see
the problem behavior with v2.1.14, but not with v2.1.6.  So, something
was introduced between these two versions that caused the problem. 
Since it works normally for you in 2.1.15 (with additional patches), I
guess it has been fixed.
