If I understand correctly, that shouldn't make a difference, since the
LilyPond installers from lilypond.org ship with Freetype and are
relocated.

Therefore an updated Freetype through MacPorts would not be used by
LilyPond unless you were compiling LilyPond from scratch.

But I'll wait to hear back from you regarding this, since I'm still
unsure about the source of the problem.
