Jacques has reported this on a variety of occasions, and we/I have always 
replied with - "this looks like the error you get when the pdf is already 
open - can you confirm that it's not?" or something like this.  We also had 
Orm Finnendahl report the same problem.  I think we need to look into the 
problem a little more, if it's preventing users getting output.

If I run Lily twice on the same file and don't close the file on Adobe 
Reader (on Windows Vista), I get this error:

I assume this is a GhostScript error?  Key thing - I seem to get an error 
code of 1 - "failed (1)", whereas Jacques seems to get 256.  I've tried to 
find what the Ghostscript errors mean, but all I can find is that they are 
supposed to be negative numbers.

The other slightly odd thing is that I get the full filename, including 
directory string after "error: failed files: " (I've deleted the directory 
details) whereas Jacques only shows the filename of the file.
