HTML output of makeinfo 4.2

Halloechen!

I had the following problems with makeinfo 4.2:

- in @verbatim environments the special HTML
  letters are not escaped.

- when going to a node, the URL always has a
  (mostly superfluous) anchor extension with
  "#..."

- no <p>...</p> tags are generated which causes
  Mozilla to ignore some paragraph boundaries.
  (Two paragraphs become one.)

- @verb ends up in a line of its own but it
  should be formatted inline.

- The implicit placement of apostrophes is not 
  consistent in dvi, html, and info output which 
  makes it difficult for the author to use them.
