Sorry, I didn't notice that part of the proposal.  I have a strong
aversion of footnotes, and don't think we should do this.

My suggestion is to have the second argument not generate output at all
in TeX and Info.  It's too hard to write the text so that it makes
sense.  Instead, it would just be used for the auxiliary
html/xml/docbook generation.

Authors have already dealt with the random-access problem however
they've dealt with it.  I don't think there's much Texinfo can offer to
help in that regard.
