Here is a patch for the Rhythm entry/Pitch overlay scheme, with a good
few improvements on the previous version. Pitch recognition is now
turned on via the microphone button on the Rhythm Entry & Overlays
toolbar. 
There is also an Overlay button, which you use if you are entering
rhythms first and pitches after. The scheme here is that you get your
rhythms entered immediately you press a duration key or click on a
rhythm pattern (or as before press Enter). Then you can add pitches to
your rhythms as before, and additionally you can add them from the
keyboard by pressing the note name (so this could be useful for minor
edits to notes in a score regardless of how it was created).
When the Overlay button is not selected the behaviour is as before.

Minor improvements: 
1) a quarter note default pattern is in place at the outset, so if you
just start typing (or playing) notes you get something, rather than the
cursor bobbing up and down.

2) In continuous mode, the cursor goes on if there is a rest at the end
of the measure (this was a bug before).

3) You can start the pitch overlay at a note after the first in the
measure (e.g. if you want to re-do part of the measure).
I'll try and do a formal ChangeLog entry asap, but I'm out of time now.
