Follow-up Comment #2:

I have found how to provoke this bug. If I set a file in 3/4 time as my
default template, then the third note entered becomes the second.
I have debugged this as far as establishing that the bug has occurred once
the second note is entered. At that point the current object should be the
second note, but it is still the first, so the third note causes an insert,
not an append. Deleting and re-starting is the work around.
