I have got this working now:

With JACK, the playback starts from the cursor or plays back the
selection if there is one. All this happens without re-creating the
MIDI, and in any case without generating external files.

I have also made it easier to unset the selection by mouse click (a long
overdue improvement).
Frustratingly savannah is down at the moment, but when it comes back I
will push all this.

I've added some design notes at

just jottings to remind me really.
