Details:
Denemo 1.8.16.
Start Denemo, switch to "Insert" mode. In menu "Notes/Rests", the first entry
is "Insert".
Close Denemo, restart Denemo: still in "Insert" mode, but in menu
"Notes/Rests", Insert is absent.
Switch to "No mode" and then back to "Insert": the "Insert" submenu in
"Notes/Rests" is back.
I may be missing something, but I found no apparent justification for what's
happening in the manual... sorry if this is not a bug.
