I can't reproduce this. I get correct behavior in all cases I've tried:
with both the old and the new exporter, html, latex and ascii.

What version of org are you using? 

Mine is

Org-mode version 7.9.1 (release_7.9.1-348-gea7baa @ 
/home/nick/elisp/org-mode/lisp/)
