--quiet does sound useless.  What about making the change indicators
only appear if you specify --verbose?

True.  A diff for a revision in which binaries changed doesn't represent
that revision accurately.  I think it would be pretty bad if someone got
a diff of my work, applied it, and found out later that the diff was
inaccurate.  So for bzr, we talked about introducing a syntax error when
using patch would lead to inaccuracies.
