In fiction, etc., yes, I suppose that's mostly so.  But in technical
writing (e.g., article titles in journals), I have seen both styles more
or less equally.  As I said, I strongly prefer the
no-gratuitious-capitalization style, and am continually frustated by
having to do it the other way in the Texinfo manual :).

Anyway.  I've put the document on the web at the "secret" location of
http://www.gnu.org/software/gnustandards/qualitycode.html.  It'll be
updated nightly when there are changes.
