[Gnumed-devel] gmI18N.sql bettered

I have wrestled with select rules to make translating tables
really transparent but, alas, PostgreSQL does not yet support
column-level select rules, so there goes ...

Anyways, in the process I have been able simplify and
robustify gmI18N.sql. The most visible change is that _() is
now called i18n() and _() is used according to its usual
semantics (fetching a translation that is). I am attaching a
lengthy tutorial which has been submitted for inclusion into
the dev guide, too.

As usual, a live example can be found in
test-area/blobs_hilbert/gmBlobs.sql + german_doc_types.sql.
