[Gnumed-devel] changes to auditing and v_patient_items

Auditing tables are now split up into audit_mark which marks a
table for audit and audit_fields which provides the fields for
auditing to descendants.

Advantage of this being that clin_root_item can have the audit
fields while still only it's descendants actually are audited
(preventing double audit on clin_root_item and any of it's
child tables). Another advantage being that v_patient_items
now also has the "last modify time" and thus an "item age"
calculated from it such that generically sorting all items by
age is possible. Thus items can easily be selected by time
range possibly also restricted by encounter/episode/issue.

(Michael, this solves what we were discussing on ICQ re
"dynamic FROM argument".)
