gmMedDoc needed to establish it's own hard (rw) connection
even for reads of BLOBs

reason being that transmitted bytea binaries (think document
archive) arrive broken when transmitted over connections with
client_encoding set to something different (say, LATIN1) than
the database encoding (which is UNICODE)

setting client_encoding is needed to correctly get locale
specific text (say, umlauts) from PostgreSQL
