I think I have come up with a solution combining the best of
both worlds:

Upon selecting a patient a new encounter is automatically
created without user intervention. The encounter type is set
to "chart review". As soon as anything is *written* to the
backend the user is asked what type the encounter should be
set to.

Does that make sense ?
