Usually, but there are times when you might refer to a group of
specialists whose office, because of schedule uncertainty, may not
right away be able to tell you which among the group's specialist will
be the one to see the patient. It is better then to put the referral
through to the group without (incorrectly) specifying either in the
outgoing correspondence, or by capturing into the local GnuMed record,
the individual who will be seeing the patient, because you do not yet
know. The referral widget should then permit a referral to a branch
(say a Division like Respirology) of an organization (say a particular
hospital's Department of Medicine) without having to specify the
individual.
While this is likely somewhat separate from the "signal" issue, I did
not know if these might be linked.
