Health issue is a long-term underlaying issue with one's
health likely to cause a few episodes of care for seemingly
unrelated problem.

Health approach seems to be a particular strategy applied to
an identified problem including but not limited to
Plan/Treatment, eg. the entire dynamics of dealing with a
problem of a patient.

That gives a view of what a particular provider did about a
potient's health over the course of several encounters.
