Not the system language but the language selected for the database content.

If someone defines a new encounter type they can name it

"Anticoag clinic"

If I then provide a translation for German "Gerinnungshemmung" I can use that 
term in my
user interface but both of us internally use the same encounter type, IOW, 
there is no two encounter
types of the same semantics with just different names in different languages.

One advantage: a search for "encounters of type anticoag clinic" can return all 
such encounters
regardless of whether I chose the type in my German user interface or you in 
your English one.
