[Gnumed-devel] 0.4 progress note creator plugin

This is how I envision the 0.4 pogress notes
creator/editor-to-be plugin to look like.

The notebook on the right with the one nondescript "Tab"
panel will hold the per-problem editors for inputting
progress notes per problem. This will again be based on
expand-as-you-type text controls but this time from the
official wxPython 2.8 sources.

Note how this nicely allows adjusting the current (!)
encounter details across all encountlets.
