1) As the address and contact are somewhat distinct from each other, may the 
label

        Primary address / contact

be

        Primary address and Contact


2) While the bottom field

        Comment

is currently stored as an attribute of this particular name, its context 
(position at bottom of the widget) is awkward and would better be relocated to 
below Nickname and Title, with the tooltip improved to

        A comment on this name for this person

        e.g.    pre-married ('maiden') name, spouse's surname
        e.g. use this alias for patient's safety
        e.g. real name NOT to be given or even acknowledged to others

Alternatively (or in addition), we could capture at the bottom of the widget

        Emergency notes
