Log message:
* src/preproc/tbl/main.cpp (process_format): Rename `x' and `X' to
`m' and `M', respectively.
Sort entries in `switch' block.
* src/preproc/tbl/tbl.man: Updated.
Sort key entries; other minor improvements.
