[Groff] Quick Fixes for Chapters 4 and 15

Larry,
        Major blunder on ch04.  Table 1
has some lines starting \. and should be
\&. although the chapter by itself formats
correctly, I guess that's a no-no even
within a table.
        Chapter 15 I just changed the .Ps/.Pe
pairs around lines 1142 and 1516 (approxmately).
Replacing them with .sp requests for some spacing
fixes the "can't break line" errors.
        I quickly tested this by creating foo.t
with:
        .so ch04.t
        .so ch15.t
and running groff with -t -s -ms foo.t > foo.ps
I got no errors and things seem to work.

        I am including two small diffs with the
fixes.
        Any other things you find, please let me
know, especially with chapter 15.  I'm not sure
if I ever considered anyone trying to do all chapters
at once, so I may have several more bugs waiting for
the unwary.

Now, since the car is finally fueled and ready
