That could be because mom doesn't provide support for pdfroff's TOC
relocation feature, in the way which -mspdf does.  Sorry, but I use ms,
not mom, so I'm not well placed to create mom bindings for pdfroff.

Yes, because you've now suppressed the extra document run, which -mspdf
uses to collate the TOC to the front of the document.
