I think this is too strict.
There should can be n "empty" lines before this.

Also:
//     (c)
// (c)   should work.
And (c) or (C)  :)

Well, you are right and I admit to be misused your nice tool :)

Of course it is. I don't see so big deal with "put it everywhere" license 
texts as far as I know the license is free. It's even against programmer's 
logic because data should be only in one place ;) Anyway, I try to 
remember project's copyright rule more carefully in future.
