Yes, you're *partially* right!
The same happened to me when I on purpose created misspelled words in
\title fields. These words and really unknown names and other words in
\title were *not* checked. For another test I wanted to check with
pstree which *spell binary actually was used and therefore I selected
and high-lighted a region of \title and \author and then invoked
ispell-region on it. It checked the fields! In AUCTeX.
Could you please check documentation and report then the bug when the
documentation does not explain why the ispell functions work so
differently?
