Then it's just a duplicate accounting request from your NAS. The
accounting information has already been entered into the database
but the NAS did not receive the accounting acknowledgment message
and therefore resent the request. Its OK if it doesn't happen too
often.

You might wish to raise the value of request-cleanup-delay in your
raddb/config. This parameter determines how long the processed
requests are kept in the queue in order to detect duplicate requests.
Normal range is 10-60 seconds. When increasing this parameter it makes
sense to also increase max-requests.
