Needed to have:



user    Auth-Type = Local,

        Password-Location = SQL

        Service-Type = Framed-User,

        Framed-Protocol = PPP



For the MD5 user file config as before, I added the the value from the md5 SQL 
function to the database field, however it didn't match.  Please provide config 
info.
