There definetly isn't a problem bootstrapping without this patch.
This patch is part of the Debian packaging.  When bootstrapping the
libio versions, I used the glibc CVS (2.2 branch) unmodified.

Given that this patch only exists in the Debian package, I'm inclined
to think that it doesn't apply to any of the cases you mentioned.

I'll ask BenC if he knows who requested it be put in (I asked Marcus,
and it's been there since before he got really involved).  I won't
have time to test the no /include case for a few days.
