Check your logs for error messages.

> notify file:
> ALL mail %s -s "CVS notification"
Try rearranging that command line to put the %s at the end:
ALL mail -s "CVS notification" %s
Also check that "mail" is the correct command name on your system and
that it has a -s switch.
