bugs in dynamic spacing

I've fixed the latter problem in CVS. Fixing the former problem is too much
work for now.  The current code does not consider the width of the
object being positioned in the Y direction.  Taking the accidental
into account would move the p from the 1st note in your example, which
is undesirable.
