[PATCH v2] SVG backend: Output a single SVG file for each page

Hello,

I have updated my patchset on Rietveld:

The only changes from the previous patchset are updates for the
documentation and source code comments.

My plan is to eventually reinstate <pageSet> and <page> when they are
supported by Inkscape, etc.  But until then, I think this rewrite of
the SVG framework offers a more reasonable solution.

Please review and comment.
