I could accept this, but not now.  The manuals
are not in any sense finished and it is more
important to complete them than it is to fiddle
with the indexing.  Also we may well change some
command names when we try to stabilise the syntax.
That would be a good time to consider how to
structure the indices.  Changing them will be a
lot of manual work - when we do it we must be
sure we don't have to do it again.
