Update Rietveld patches

The items below are the result of a search on Rietveld for open items associated with the lilypond trunk.  In some cases, the tracker item has been closed, so the Rietveld item should also be closed.  In others, work continues but it would be good to cross-reference the tracker item in the Rietveld item summary or title.

At the moment, I have no simple way of scanning Rietveld items which do not have their base URL set to
http://git.savannah.gnu.org/gitweb/?p=lilypond.git/trunk/ and I would be most grateful if the devels who have patches on Rietveld would find a few moments to update them by adding the base URL and if at all possible, the tracker number to which the patch belongs.
