The easiest way to obtain dynamics in parentheses is to use
text markup. Something like
c4_\markup{ ( \dynamic mp ) }
or maybe you prefer
_\markup{ \bold { ( \dynamic mp ) }

For note heads, take a look at the example called molecule-hacking.ly
in the Regression Tests document included in the documentation for
your version of LilyPond (please always tell what LilyPond version
you use when you send a question, otherwise you might get an answer
that's irrelevant to you, I have assumed version 2.4 above).

wishlist: in the piano music i am writing, sometimes the same note
appears in more than one part and it's useful to be able to put one of
them in parentheses so the player knows that it is intended that only
one finger plays the note (an example from published music can be
found in ravel's "gaspard de la nuit", edition peters (ed. nichols)).
