Sorry, I should have posted a minimal example. Here is one:

lilypond-book --pdf --o=out  test.tex

pdflatex test.tex

The squared boxes are not the same. This is not a surprise, but it could
be improved. But really, the problem is exactly the same in metapost
(http://en.wikipedia.org/wiki/MetaPost).
