Remove the braces around the note:
\repeat tremolo 4 d8
This buglet (see
http://code.google.com/p/lilypond/issues/detail?id=348) was fixed in
2.13.18. I'm afraid it won't be backported to 2.12, but given the
workaround above, that shouldn't matter too much. :)
