No, but on certain primitive operating systems there is a limit on
the length of file names, so if one wants portability this constrains
their length for everyone.

These were never envisaged, so something is sure to break if you
have them.

Yes: Base, Slope, Bold, and BoldSlope.  Every family should have
these four at least, but using fontdef you can map them to
whichever actual face you like.

> About recoding

NoRecode means that the encoding of the font resident in your print
device is identical with the one in the .LCM file, and hence Lout
does not need to send this encoding to the printer.  It has nothing
to do with the number 128.  If in doubt, choose Recode.
