Have you looked at ikiwiki?  I have, and creating a monotone plugin
doesn't seem too hard, it's on my TODO list (although currently
dormant, because I've other things that have higher priority.

I have no idea whatsoever how ikiwiki would integrate with something
like Trac or similar.

Untrue, monotone is registered as a project in savannah.nongnu.org,
and the bug tracking system there is used for it.  Too few are really
looking there, though, including myself (because I disilked the
interface, but it looks like it's better now).
