Can this ever practically happen? now is a 64b ns timer so it should
take a very long time for it to wrap around.

This has no prescaler awareness. Modifying the timer value will modify
the post pre-scale value so this will having to account for the
prescaler somehow. value here will be in terms of after-prescaler
clock cycles, whereas tick_offset is in terms of before-prescaler
cycles.
