Yes, I have booted my kernel with the intel_iommu=on. (I don't remember how to check that now though ^^)

Assuming you are on an intel box, have you booted your kernel with intel_iommu=on ?

Why does opening /dev/vfio/vfio fail ? Can you please confirm that you have read/write permissions as the user you are trying to run ?
