Eventually the form should be a HTML form which stores the data directly
into the database. Better parseable would definitely be nice for the
time it'll take to get the application and testing (and whatever else)
databases up and running.

With database I mean some SQL-based database like MySQL, with online
search capabilities. I want to be able to do complex queries which are
not available with spreadsheets.
