Unless I'm mistaken, you ask users to advise the author (you) for any
modifications made to the software. Is it a requirement or a request?
Note that it cannot be compatible with the GPL if it's a requirement.

 In order to release your project properly and unambiguously 
under the GPL, please place copyright notices and permission-to-copy
statements at the beginning of every file of source code.

In addition, if you haven't already, please copy a copy of the plain
text version of the GPL, available from
(http://www.gnu.org/licenses/gpl.txt), into a file named "COPYING".

The GPL FAQ explains why these procedures must be followed.  To learn

Also, in the links you provided, I'm unable to find a ligne of code,
apart from the HTML code. As result, I'm not sure to exactly
understand what is your project. Would you like to provide more
details.
