I've got it running on a RH9 testing box with MySQL 4.0.14
(devel,client,shared-compat). The configure options I used was:
'--enable-mbox-maildir' '--enable-fcntl-locking' '--disable-dotfile-locking'
'--enable-auth-mysql' '--disable-auth-pam'
'--with-mysql-include-dir=/usr/include/mysql' '--with-mysql-lib-dir=/usr/lib'
