I've used http://www.pcbpool.com/ppuk/service.html for a single small 
order: 10 off of 1.5 sq inches, silk screen and solder resist cost £70.

They don't do assembly themselves, but they have a database of 
assemblers and there are more links on their Link Yourself page.

PCB Pool is rather small for your requirements, but will quote your 
quantities; 5000 off with a 11 working day turn-around would cost EUR 
3516.00 (+shipping) (73mm x 32mm, dbl sided, position print, 
electrically tested)
