[PATCH 0/5] {maint} Use %empty in the reports

The following changes ensure that %empty is output for rules with
an empty right-hand side in all our report formats.  There's a lot
of code duplication there, but fighting it in C would probably
obfuscate the code too much.  I wish Bison were in C++.
